You are receiving this email if your school health records are not complete.  Please turn in all forms to the school nurse during the "Get acquainted" days. 
 
Please call me if you have any questions and are unsure about what may be missing.  The Florida Department of Health is quite specific about what is required in order for a student to attend class.
